9. Stretch Manufacturer
10. Lienholder(s).

2. A copy of the vehicle registration for each unit that coverage is requested.

3. A copy of your MDOT certificate of authority or a copy of your MDOT application.

4. A complete list of Drivers which should include:
1. Full Name
2. Date of Birth
3. Drivers License Number
4. State of Issue
5. Date of Hire
